Board of Certified Professional Midwives Licensing Requirements

Licensing Guidelines & Instructions

Submit the following to the South Dakota Board of Certified Professional Midwives: 

  • Completed Application for CPM License Form
  • Official transcript - Transcripts must be sent by the institution that conferred the document
  • Official copy of your diploma or certificate - Diplomas must bear the school seal and Dean, Registrar, or equivalent signature.
  • Letter of good standing from every state where you currently hold licensure. 
  • Bridge Certificate from NARM and description of your educational experiences - If non-MEAC school graduate
  • Criminal Background Check - Pursuant to SDCL 36-9C-12 each applicant for initial licensure is required to submit a full set of fingerprints to obtain a state and federal criminal background check. Upon request or receipt of your completed application, the South Dakota Board of Certified Professional Midwives will provide you with a background check packet which will include SDBCPM-specific fingerprint cards. You must use the agency-specific cards. 
  • $1000 Fee* - Payment should be in the form of a money order or personal check payable to the South Dakota Board of Certified Professional Midwives. Fees are refundable in very limited circumstances and must accompany the form. A $40 fee will be charged for any insufficient check written.

*The $1000 CPM licensure fee will include two components: a $500 nonrefundable portion and a $500 portion that is refundable if the application is withdrawn or denied.

Fill out the CPM Application Form

Licensure renewal information and fees must be received by the South Dakota Board of Certified Professional Midwives office by your license expiration date or your license will lapse. It is illegal to practice professional midwifery in South Dakota without an active CPM license.  You are responsible to maintain licensure whether or not you receive a renewal notice.  

All forms and fees must be postmarked on or before your expiration date to avoid lapsing.

To RENEW your CPM license, submit the following to the South Dakota Board of Certified Professional Midwives office:

  • Completed Application for CPM License Renewal Form

  • $1500 Fee: Payment should be in the form of a money order or personal check payable to the South Dakota Board of Certified Professional Midwives. Fees are non-refundable and must accompany the form. A $40 fee will be charged for any insufficient check written.

CPM License Renewal Form

Submit the following to the South Dakota Board of Certified Professional Midwives:

  • Completed Student Midwife Application form

  • Completed Student Midwife Provisional License and Preceptor Agreement

  • Official letter of proof of enrollment from MEAC-approved program.

Student Midwife Application Form

Guidelines

  • Midwifery practice shall not be permitted until this Preceptor Agreement has been approved by the Board.

  • The Provisional Licensee shall only provide services under the ongoing direct supervision of the Board-approved preceptor.

  • The Provisional Licensed Student Midwife shall adhere to the South Dakota statutes 36-9C-1 through 36-9C-37, and the Administrative Rules promulgated by the Board.

  • The Preceptor and the Provisional Licensed Student Midwife shall provide full disclosure of the supervised nature of their work to every client before or during the first appointment.

  • The Preceptor shall not allow the Provisional Licensed Student Midwife to provide student midwifery services independently of their Preceptor.

  • The Preceptor shall keep records verifying the training and evaluation of the Provisional Licensed Student Midwife, including the precise nature of services rendered.

  • The Preceptor shall be identified on all reports and correspondence of a professional nature, excluding disciplinary correspondence with the Board.

  • The Preceptor assumes professional and ethical responsibility and may be sanctioned by the Board for all acts and omissions of the Provisional Licensed Student Midwife within the scope of the supervision.
