A written complaint may be filed by any person claiming that a certified professional midwife licensee or an applicant for a license has engaged in or is engaging in conduct consisting of grounds for disciplinary action. The complaint form is required to contain the name of the applicant or licensee and full detail of the conduct alleged to be in violation.
