    Order national standardized examinations, parts I, II, and, if applicable, part III of the National Board of Podiatric Medical Examiners as well as a Disciplinary Action Search from

    Fill out verification form for Podiatric Residency or proof of completing this requirement in another state.

  7. Verify Licensure in Other States

    Verify licensure from each state you hold or have held licensure in. You may complete our form or submit your own form if it contains the same information. You must submit directly to our office via mail or email.
