Board of Podiatry Examiners Licensing Requirements

  1. Apply

    Fill out and mail an application that is signed and notarized.

  2. Fees

    Send your application fee to the Board office with your application. Fee is $500.

  3. Medical Transcripts

    Order Podiatric Medical Transcripts and send directly to the Board office via mail or email.

  4. Undergraduate Transcripts

    Order undergraduate transcripts and send directly to the Board office via mail or email.

  5. Standardized Examinations

    Order national standardized examinations, parts I, II, and, if applicable, part III of the National Board of Podiatric Medical Examiners as well as a Disciplinary Action Search from fpmb.org

  6. Verification of Residency

    Fill out verification form for Podiatric Residency or proof of completing this requirement in another state.

  7. Verify Licensure in Other States

    Verify licensure from each state you hold or have held licensure in. You may complete our form or submit your own form if it contains the same information. You must submit directly to our office via mail or email.
