The board has established the following fees for testing and licensure.
- Hearing Aid Dispenser Application for Licensure - $200
- Hearing Aid Dispenser by Endorsement Application - $200
- Provisional Hearing Aid Dispenser Application - $100
- Hearing Aid Dispenser Renewal Fee (annual) - $200
- Late Fee - $50
- Audiologist Application for Licensure - $200
- Audiologist by Endorsement Application - $200
- Provisional Audiologist Application - $100
- Audiologist Renewal Fee (annual) - $200
- Late Fee - $50
All fees are non refundable.
