The board has established the following fees:
- Director Application for Licensure — $225
- Director Renewal Fee (annual) — $225
- Establishment Application — $350
- Establishment Renewal (annual) — $350
- Crematory Application — $100
- Crematory Renewal — $100
- Trainee Registration Fee — $25
All fees are non-refundable.
