  • Promote health and reduce risk factors to decrease the onset of chronic health conditions.
  • Use programs and policy to build partner capacity and tailor interventions that help eliminate disparities.
  • Transform communities by promoting social, environmental, policy, and systems approaches that support healthy living for individuals, families, and communities where they live, work, and play.

SD DOH Chronic Disease Programs

Healthcare Referrals

Providers play a vital role in preventing and controlling chronic diseases. In South Dakota, that includes referring tobacco-using patients to the SD QuitLine, providing general health-related information to patients, ensuring screenings for various chronic diseases take place, using health information technology to ensure the overall quality of care, and linking patients to other community health and education resources.

Chronic Conditions

Better Choices, Better Health SD
  • For patients with a chronic condition
  • Free workshops held at in-person locations across South Dakota and virtually through Zoom

Cancer

All Women Count!
  • For women ages 30 to 64
  • Helps cover the cost of breast and cervical cancer screening and diagnostic services

Diabetes

National Diabetes Prevention Program (NDPP)
  • For patients who have prediabetes or are at risk for developing Type 2 diabetes
  • Year-long program led by a lifestyle coach that helps participants learn new habits, gain new skills and build confidence to eat better and become more active
Diabetes Self-Management Education and Support Program (DSMES)
  • For patients who have recently been diagnosed, annually, when complicating factors occur, or during transitions in care

Physical Activity

Park Rx
  • For all patients
  • Prescribe physical activity. Patients take the Rx to any state park and enjoy a free day in the park or a discounted annual pass.

Tobacco

South Dakota Quitline
  • For patients 13+ interested in quitting cigarettes, chewing tobacco, or e-cigarettes
  • Free support, medication, and phone coaching

Opioid

South Dakota Opioid Resource Hotline
  • For patients with Opioid Use Disorder, their friends, family, and providers
  • 24/7 hotline answered by trained crisis workers that can assist in finding local support, treatment, and resources

The Office of Chronic Disease Prevention and Health Promotion and partners offer a variety of funding opportunities to assist organizations working to reduce the burden of chronic disease in South Dakota.
