Renewal Reminder Email
- You’ll receive an email 45 days before your card expires.
- You will need an appointment with your medical provider to renew your certification. Do not delay this step because the approval process may take a couple of weeks.
- After your provider renews your certification, you will then fill out your patient application with your previous login credentials.
- If you forgot your password, click ‘Forgot your password?’ to go through the process of resetting it. When resetting your password, you must open a new internet browser tab to log into your email inbox before going back to the initial tab to enter your verification code. Once your application is pending, it is in the queue to be reviewed by Department staff.
- If you have already submitted your renewal, you may disregard the email notice.
Renewal Approval Email
- When the renewal application is approved, you will receive an approval email, and your card
will be printed and placed in the mail. - Once you receive your updated card, you must shred and dispose of your previous card.
Important Things to Remember
- Make sure all information is correct before you submit your application. If you log in and make any changes to your application after submission, the system automatically moves the application to the bottom of the list.
- Unless you intend to home cultivate and have the photo evidence ready, do not click ‘home cultivation’ on your application. If you are unclear about what ‘home cultivation’ is, please research the requirements before completing your application.
- NON-RESIDENT CARDHOLDERS:
- List the names of two dispensaries that you intend to purchase on your application. You will only be able to shop at the dispensaries listed on your non-resident card.
- A copy of the original certification from your provider, including the debilitating medical condition and the provider’s signature, dated within the last 12 months, must be provided. Photocopies of your out-of-state medical card are not accepted.
- LOW-INCOME CARD HOLDERS:
- Copies of benefit letters must be from the agency issuing your benefits. Written statements from patients are not accepted. Please see the patient's step-by-step guide for a list of documentation that is acceptable for proof of low-income status.
