Electronically submit worksheets and applications for the following services:
Parents Worksheet
Expectant parents may complete the Parents Worksheet for electronic submission to the hospital.

After your baby is born, the hospital will use the information on your completed Parents Worksheet to officially register the birth certificate within 7 days of birth.

Marriage License Application
A couple wishing to get married in South Dakota can complete the Marriage License Application for electronic submission to the Register of Deeds office.

- Submit when finished
After you submit the application, both applicants must visit the Register of Deeds office to:
- Finalize the issuance of their marriage license
- Sign in the presence of the Register of Deeds
- Provide valid photo identification and proof of age before issuance of the marriage license:
- Passport.
- Federal, state, military, or tribal photo identification.
- Certified birth certificate, along with a current school or employment photo ID.
- Certified birth certificate, along with a completed U.S. Department of the Treasury Form 4029.
The Register of Deeds will use the completed application to create and issue a South Dakota marriage license that is valid for 90 days and must be returned to the Register of Deeds within 10 days of marriage.
