Nursing Facility Reports
South Dakota's nursing facilities are listed below by community. The latest inspection report is provided for each facility; the date indicates the date that the survey was conducted. On average, each facility is surveyed for recertification once a year (9 to 15-month intervals) by the South Dakota Department of Health, Office of Health Care Facilities Licensure and Certification.
The inspection reports are referred to as the Statement of Deficiencies (Form 2567). The left-side column of the report lists deficiencies identified by the survey team while the right-side column details the facility's Plan of Correction to address those deficiencies. The Life Safety Code report and state licensure reports are also included, using the same column format.
Explanation of Deficiency Scope and Severity
Complaint surveys are provided as a separate report in the listing below. Complaint surveys may be conducted at the same time as the recertification survey and will be noted in the report. If no date is listed in the complaint survey column, there have been no complaint surveys.
Recertification and complaint surveys will be retained on this site for three years.
