Will you be responsible for either the storage and handling of vaccine or performing vaccinations? If so, after you complete the required training, you will need to submit a signed attestation indicating such.

Required Training Attestation Form:

Please print and sign the training attestation form and then bring the signed attestation with you to your vaccine volunteer event. The event coordinator will require this before you can handle vaccines or administer vaccinations.
