Registry Information:
The South Dakota Tuberculosis (TB) Control Program maintains a registry of patients with active TB disease and latent TB infection. The registry includes comprehensive information such as TB test results, chest X-ray reports, and TB treatment details.
Sharing of Information:
Patient information from the registry can be provided to the patient's medical provider or a licensed healthcare worker at a facility. This sharing is necessary to comply with facility TB screening requirements.
Accessing Personal TB Records:
Patients who want a copy of their TB record must sign the TB Control Program Release of Information Form (HS450). Signing this form is a requirement before their record can be released.
