Heart Disease & Stroke Funding Opportunity

The South Dakota Department of Health (SD DOH) Heart Disease and Stroke Prevention Program (HDSPP) is offering funding to support evidence-based strategies that improve the prevention and management of cardiovascular disease (CVD) across South Dakota. Heart disease remains the leading cause of death in the state, and high blood pressure and high cholesterol continue to be major risk factors impacting adult health statewide.

Through this funding opportunity, SD DOH will support organizations implementing sustainable, systems-level approaches that improve hypertension management, care coordination, and long-term cardiovascular health outcomes.

Organizations may apply to implement one or both of the following priorities:

Priority 1: Self-Measured Blood Pressure (SMBP) Program
Supports the implementation or expansion of self-measured blood pressure monitoring programs through partnerships with healthcare facilities and clinics. These programs help individuals with hypertension manage their condition by monitoring blood pressure at home with clinical support.

Priority 2: Care Coordination Program
Supports a patient-centered care coordination model that provides comprehensive and coordinated management for individuals with multiple chronic conditions. This priority emphasizes identifying and addressing non-medical social and environmental factors that impact cardiovascular health, including transportation, food access, housing, and other support needs.

Facilities awarded funding will receive financial support and technical assistance to create or expand sustainable programs that promote self-management and lifestyle changes for individuals at increased risk for cardiovascular disease, with a focus on hypertension management.

Funded projects must demonstrate a systems-level approach that supports lasting organizational change, show strong potential for sustainability beyond the funding period, and contribute to improved cardiovascular health outcomes. Programs that address health disparities and serve high-risk populations are encouraged.

Available Funding

Applicants may request:

  • Up to $7,500 to implement Priority 1 (Self-Measured Blood Pressure Program)
  • Up to $30,000 to implement Priority 2 (Care Coordination Program)
  • Organizations may apply for funding to support both priorities.

Eligibility

Eligible interventions must serve South Dakota residents or tribal communities and target adults ages 18 to 85. Certain populations are excluded, including individuals receiving hospice services, those with end-stage renal disease, and residents of long-term care facilities. Organizations receiving federal funding, including the Indian Health Service and Federally Qualified Health Centers, are eligible to apply.

  • Application Submission Deadline: February 25, 2026, by 5:00 p.m. CT
  • Tentative Award Notification: March 25, 2026
  • Project Period: June 1, 2026, through May 31, 2027
