Nexus SD Implementation Award RFA

The South Dakota Department of Health is proud to announce the Nexus SD Implementation Award, designed to address critical health disparities through collaboration and innovation. This funding opportunity supports organizations dedicated to improving health outcomes by addressing social determinants of health in underserved populations. Nexus SD provides a streamlined platform for healthcare and social service providers to coordinate care, ensuring individuals receive the support they need. By participating in this initiative, organizations can enhance their services and access resources that improve both individual and community health outcomes.

Eligible organizations, including 501(c)3 nonprofits, government entities, and certain for-profit businesses, are invited to apply for funding of up to $10,000. The application process, open until October 18, 2024, will prioritize applicants serving high-need populations in South Dakota. Awardees will receive funds based on milestone achievements, such as onboarding new Nexus SD members and providing services to individuals in need. This initiative aligns with the CDC’s Healthy People 2030 goals of promoting health equity and addressing social determinants of health, reinforcing South Dakota’s commitment to improving public health.

Application Submission Deadline: October 18, 2024, 5:00 pm CT

Tentative Award Notification: December 1, 2024

Project Period: January 1, 2025 - May 31, 2025
