CHW Immunization Program RFA

OPEN February 12, 2025 | Application Due Date: March 12, 2025

The South Dakota Department of Health (SD DOH) Immunization Program, in partnership with the Centers for Disease Control and Prevention (CDC), is seeking applications from organizations that employ Community Health Workers (CHWs) or Community Health Representatives (CHRs) to enhance vaccination rates and vaccine education. Funded under a CDC cooperative agreement, this initiative supports CHWs/CHRs in reviewing immunization records, promoting vaccinations, addressing patient concerns, scheduling appointments, assisting with transportation, and providing follow-up care. The goal is to increase vaccine access and confidence in South Dakota communities.

  • Application Deadline: March 12, 2025, by 5:00 p.m. CT
  • Tentative Award Notification: May 1, 2025
  • Project Period: June 1, 2025 - May 31, 2026
  • Funding Available: Up to $15,000 per Community Health Worker position
  • Number of Anticipated Awards: 10
