- State Written Examination — $50
- Director / Embalmer Renewal Fee (annual) — $125
- Service Renewal Fee (annual) — $50
- Trainee Registration Fee — $25
- Establishment Application — $250
- Establishment Renewal (annual) — $250
- Trust Filing Fee (annual) — $5
- Crematory Application — $100
- Crematory Renewal — $100
All fees are non-refundable.
