The Board of Certified Professional Midwives charges fees for licensing, renewal, and other services listed below. Payment should be in the form of a money order or personal check payable to the South Dakota Board of Certified Professional Midwives. Fees are non-refundable unless otherwise noted and must accompany the form. A $40 fee will be charged for any insufficient check written.
- Initial Licensure Fee: $1000
The $1000 CPM licensure fee must accompany the application form and will include two components: a $500 nonrefundable portion and a $500 portion that is refundable if the application is withdrawn or denied. - Biennial Renewal Fee: $1500
The $1500 fee must accompany the license renewal form
- Lapsed License Fee: additional $500
- Lapsed License Fee: additional $500
- Student License Fee: $500
The one-time $500 fee must accompany the student application form and preceptor agreement. - Inactive License Fee: $100
The $100 fee must accompany the inactive status form. - Birth Delivery Fee: $100
The $100 fee must be accompanied by the birth reporting form, within 30 days of delivery. - Transcript Fee: $25
