Fees
The board has established the following fees for testing and licensure.
- Application for Licensure - $500.00
- Reciprocity Application - $500.00
- Licensure fees Applicants granted licensure will pay - $250.00
- Licensure fees Reciprocity - Applicants granted licensure will pay - $250.00 on licensure approval
- Renewal Fees (annual) - $150.00
- Replacement license - $10.00 and a written request
- Corporation Registration - $50.00
- Corporation Annual Renewal - $10.00
- PM Lexis Fee direct to PM Lexis.
All fees are non refundable.
