The board has established the following fees for testing and licensure.
- Director/Embalmer Application for Licensure - $100.00
- State Written Examination - $50.00
- Director/Embalmer Renewal Fee (annual) - $100.00
- Service Renewal Fee (annual) - $100.00
- Trainee Registration Fee - $25.00
- Establishment Application - $200.00
- Establishment Renewal (annual) - $200.00
- Trust Filing Fee (annual) - $5.00
- Crematory Application - $100.00
- Crematory Renewal - $100.00
All fees are non refundable.
