Certificate of Birth Resulting in Stillbirth

The Office of Vital Records issues Certificates of Birth Resulting in Stillbirth (CBRS) for fetal deaths of any fetus that has attained a gestational age of no less than 20 completed weeks.

A CBRS may be issued to either of the parents listed on the record. The fee to obtain this record is $10.

CBRS may be requested through the mail or in person at Vital Records.

Before applying for a Certificate of Birth Resulting in Stillbirth please note:

  • This is considered a commemorative certificate. To request a Report of Fetal Death (comparable to legal death certificate), complete the Vital Records Request form and submit it with $15 and a copy of your state-issued photo ID.
  • Certificates cannot be faxed or e-mailed.
  • The fee is required prior to any search.

Mail request to the State Office, 221 West Capitol Avenue, Pierre, SD 57501.

Processing time is 3-5 business days. You may submit your request in a prepaid priority envelope for expedited processing (optional).

Mail requests require a completed application, signed in front of a notary OR with a clear copy of a photo ID, along with the fee.

If you do not provide a clear copy of your valid photo ID (State, Tribal or Federal) or a notarized application, you will be required to submit two additional forms of identification:

  • Copy of Social Security Card
  • Utility bill with current address
  • Bank statement with current address
  • Copy of car registration or title with current address
  • Pay stub (must include your name, social security number, and the address of the business)

Order in person at the State Office, 221 West Capitol Avenue, Pierre, SD 57501. You will be required to complete and sign an application, pay the appropriate fee, and provide proof of identity.
