Diabetes-Specific Community Health Worker Program Request for Applications
The South Dakota Department of Health’s Diabetes Program is pleased to announce the release of the Diabetes-Specific Community Health Worker (CHW) Program Request for Applications (RFA).
This funding opportunity will provide one medical/clinical organization with funding to hire and train individuals to work with patients with Type 2 Diabetes and prediabetes. The organization awarded the funding opportunity will receive technical assistance and support from the SD Diabetes Program and the Community Health Worker Collaborative of South Dakota (CHWSD). ELIGIBILITY CRITERIA: Eligibility includes organizations, based in the state of South Dakota, that are a medical/clinical program/organization (such as a hospital, medical clinic, tribal health clinic, or FQHC) that serve patients with diabetes and prediabetes. Project activities must occur within the geographical borders of South Dakota and target residents within South Dakota.
APPLICATION DEADLINE: 5:00 pm CT on Friday, August 23, 2024.
Preliminary notice of award will be provided by August 30, 2024, with a contract to begin in September 2024.
ANTICIPATED AWARD AMOUNT: Applicants may request up to $49,900 for the project period of September 2024 – May 31, 2025.
PROJECT PERIOD: September 2024 - May 31st, 2025
APPLICATION REQUIREMENTS: In addition to providing basic organizational information, applicants will also be required to provide narrative responses (1-2 paragraphs each) regarding the population of focus, organizational overview, and program evaluation. Additionally, applicants will be required to submit a draft project budget, a W-9 form, proof of insurance, and a Certificate of Completion of the CHW Toolkit.
CHW TOOLKIT: At least one representative from the applying organization must complete the South Dakota Community Health Worker Planning and Sustainability Toolkit and submit a certificate of completion as part of the RFA.
