TB Registry
The South Dakota TB Control Program maintains a registry of patients reported with active TB disease and latent TB infection. This registry contains the information reported on the patient including TB test information, chest X-ray and TB treatment information. This information may be provided to the patient's medical provider or a licensed health care worker at a facility when the information is necessary to comply with a facility TB screening requirement. The patient's information may be mailed or faxed but will be sent directly to the licensed staff person requesting the information. Patients who wish to obtain a copy of their TB record must sign the TB Control Program Release of Information Form (HS450) before their record will be released.
