TB Registry

The South Dakota TB Control Program maintains a registry of patients reported with active TB disease and latent TB infection. This registry contains the information reported on the patient including TB skin test information, chest X-ray and TB treatment information. This information may be provided to the patient's physician or a licensed health care worker at a facility when the information is necessary to comply with a facility TB screening requirement. The patient's information may be mailed or faxed but will be sent directly to the licensed staff person requesting the information. Patients who wish to obtain a copy of their TB record must sign the TB Control Program Release of Information Form (HS450) before their record will be released. In order to access information in the South Dakota TB Registry, please call 605-773-3737 or 1-800-592-1861 (in-state only).
