Physical Therapist Assistant (RPTA)

TO: Applicants

 

The application must be completed in its entirety and submitted with the appropriate fee or it will be returned.

 

The applicant is responsible to request verifications from each primary source (there may be fees for this service) to be sent directly to the South Dakota Board

 

The Board must receive all primary source verification and the background investigation must be completed prior to licensure or certification

Physical Therapist Assistant Certification Information Documentation CANNOT be faxed or emailed.

Endorsement - applicants who have taken and passed exam:

  1. Complete application form; include photo
  2. Request that the Federation of State Boards of Physical Therapy (FSBPT) send scores directly to the SDBMOE office.
  3. Submit fee of $60;
  4. Request verification of all professional licenses, permits, certificates; whether active or inactive  from each state board to be sent directly from them to the SDBMOE office = Verify licenses
  5. Request verification of graduation (attach picture) to be sent directly from the PT program to be sent directly to the SDBMOE office. Verify graduation from physical therapy assistant program 
  6. Complete the supervising physical therapist application.
  7. Verification of Employment and Privileges Verification of Employment
  8. Foreign trained physical therapist assistant* see below

Examination - for those applicants who STILL NEED to take the exam:

  1. Complete application form; include photo
  2. Submit fee of $60;
  3. Request verification of graduation (attach picture) to be sent directly from the PT program to be sent directly to the SDBMOE office. Verify graduation from physical therapy assistant program

Please be aware that you may not engage in any type of practice, even under supervision, until you obtain your license or certificate and have received your licensure or certification card from this office.
