Physical Therapist (RPT) TO: Applicants

• The application must be completed in its entirety and submitted with the appropriate fee or it will be returned.
• The applicant is responsible to request verifications from each primary source (there may be fees for this service) to be sent directly to the South Dakota Board.
• The Board must receive all primary source verification and the background investigation must be completed prior to licensure or certification.

Physical Therapist Licensure Information: Documentation CANNOT be faxed or emailed

• Endorsement - applicants who have taken and passed exam:
  • Complete application form; include photo
  • Request that the Federation of State Boards of Physical Therapy (FSBPT) send scores directly to the SDBMOE office.
  • Submit fee of $60;
  • Request verification of all professional licenses, permits, certificates; whether active or inactive  from each state board to be sent directly from them to the SDBMOE office = Verify licenses
  • Request verification of graduation (attach picture) to be sent directly from the PT program to be sent directly to the SDBMOE office. Verify graduation from physical therapy program 
  • Verification of Employment and Privileges Verification of Employment

• Examination - for those applicants who still need to take the exam:
  • Complete application form; include photo
  • Submit fee of $60;
  • Request verification of graduation (attach picture) to be sent directly from the PT program to be sent directly to the SDBMOE office. Verify graduation from physical therapy program
