Physician Assistant (PA) License

TO: Applicants for Physician Assistant

Select the type of license and follow those instructions completely. The application must be completed in its entirety and submitted with the appropriate fee or it will be returned to you. 

The applicant is responsible to request verifications from each state board, the college of graduation and employment history.

Physician Assistant Licensure Requirements:

  1. Complete application form and mail with $75 non-refundable fee to Board office;
  2. Request Verification of Graduation form to be sent from program directly to Board office
  3. Request Verification of Licensure form (usually a fee to do this) to be sent from other states directly to Board office
  4. Request Verification of Employment form (past or present) sent directly to the Board office;
  5. Request PANCE Scores from NCCPA to be sent from NCCPA directly to the Board office
  6. A copy of  a signed practice agreement faxed to the Board office where it is validated and approval letter faxed back to PA @ work office (prior to practicing as a licensed SD PA)

 

At discretion of Board these may be required for Physician Assistant Licensure - applicant will be informed in writing:

  1. Applicant in person interview at the Board office or with a Physician Assistant Advisory Committee (PAAC) member
  2. Signed affidavit in event that practice agreement not available prior to licensure
