If you receive an application or associated forms from a source other than directly from this office or its official web site, the documents may be outdated or not official versions. Application forms will be rejected if not on the current version.
Please note that an initial registration will vary in duration dependant upon its issuance date. This initial registration will expire on December 31 of an odd numbered year. It is the responsibility of all licensees to renew their registration before December 31 of the odd numbered year . Failure to renew by December 31of every odd numbered year shall result in automatic lapse of the registration by statute. Registrations which have lapsed are not automatically entitled to renew. Renewal Process — go back to the Board of Medicine Home these applications are only available in the fall every other odd year and will be listed under Renewal Link when available. Do not rely on notification; use the web site to obtain renewal form.
Reinstatement Process — send written explanation of why your registration was allowed to lapse with your original signature to the Board office. Once your statement is evaluated, you will receive written instructions as to whether your previous registration can be reinstated. At that time, you will also receive written instructions as to what is required to reinstate and renew your registration.
