The application must be completed in its entirety and submitted with the appropriate fee or it will be returned.
The applicant is responsible for all applicant initiated primary source verification
Please be aware that you may not engage in any type of practice, even under supervision, until you obtain your license and receive your licensure card and certificate from this office.
**UNLICENSED PRACTICE AS AN ATHLETIC TRAINER IS A CRIMINAL OFFENSE IN VIOLATION OF SDCL 36-29. THE BOARD INTENDS THIS NOTIFICATION TO SERVE AS NOTICE UPON YOU THAT UNLICENSED PRACTICE IS UNPROFESSIONAL CONDUCT, A VIOLATION OF THE ABOVE CODIFIED LAWS, AND GROUNDS FOR DENIAL OF LICENSURE.**

Athletic Trainer License
- Complete application form and file with Board office .
- Complete Team/Referring Physician Protocol
- Submit fee of $100.
- Request Verification of Licensure to be sent directly from state board(s) to Board office, there may be a fee involved.
- Submit Verification of Employment .
- Request Verification of Graduation from Athletic Trainer Program to be sent directly from program to Board.
- Request Exam scores to be sent directly to the Board by the National Athletic Trainer’s Association Board of Certification, Inc. Copies unacceptable.
- Interview with one subcommittee member and present the following original credentials:
- College Credentials (original Athletic Trainer Diploma)
- NATA Certification
Athletic Trainer Temporary Permit
**Please be aware that a temporary permit is valid for only 180 days. Also, please be aware that there is no grace period and you may not engage in any type of practice, even under supervision, until you obtain at least a temporary permit and receive notification from the Board.
- Complete application form and file with Board office .
- Request Verification of Graduation from Athletic Trainer Program to be sent directly from program to Board.
- Submit Physician Protocol providing supervision by a licensed physician, licensed physical therapist, or licensed athletic trainer. (Please be aware that in order to convert your temporary license to an active license, you must have a licensed South Dakota physician complete the physician protocol).
- Submit a statement indicating the name and address of employer during time of temporary permit.
- The Board will contact you directly should any additional verifications or materials be required.
