South Dakota Nursing Facility Survey Reports

South Dakota's 111 nursing facilities are listed below by community. For each facility the latest inspection report is provided in ADOBE format; the date indicates the date that survey was conducted. On average, each facility is surveyed for recertification once a year (9 to 15 month interval) by the South Dakota Department of Health, Office of Health Care Facilities Licensure and Certification.

The inspection reports are referred to as the Statement of Deficiencies (Form 2567). The left-side column of the report lists deficiencies identified by the survey team while the right-side column details the facility's Plan of Correction to address those deficiencies. The Life Safety Code report and state licensure reports are included as well, using the same column format. (An explanation of the Scope and Severity deficiency codes noted in the reports can be found here.)

If a complaint survey has been conducted that survey is provided as a separate report in the listing below. A complaint survey may be conducted at the same time as the recertification survey and will be so noted in the report. If no date is listed in the complaint survey column, there have been no complaint surveys.

Recertification and complaint surveys will be retained at this site for three years.
