The South Dakota Ryan White Title II CARE Program is funded by an annual federal grant. Its goal is to assist low-income, HIV infected individuals with the cost of specific health care needs. Benefits of the program are determined by federal guidelines and a state advisory council made up of representatives from health care, those with HIV disease, support groups and state agencies. The Department of Health administers the program. Due to limited funding, applicants may be placed on a waiting listing until the program can serve them.

Complete and submit the application form included in the current Ryan White Title II CARE Program Information and Application Packet (use ADOBE). Applicants will be notified in writing whether they are eligible for services through the program or whether they will be placed on a waiting list for services. One service available to those on the waiting list is case management services. A case manager can help you find other resources to assist you.
