Requesting a South Dakota Immunization Record

1) Check first with the doctor, Community Health or Public Health Alliance office where the immunization was given. Current records for most preschoolers should be available from these sites. Limited records for older children and adults may also be available if the immunizations were given recently.

2) Contact the South Dakota Department of Health, Immunization Program, by phone at 1-800-592-1861 (in SD only) or email. The department maintains microfilm files of immunization records required for school entry (both kindergarten entry and transfer into a school system) from the 1976-1977 school year through the present.
