
Connecting the Dots for Children and Youth With Special Health Care Needs
Childrens Special Health Services (CSHS) assures that children and youth with chronic medical conditions receive the full range of services they need, that services are not duplicated, and that providers communicate with families and each other about patients' care. CSHS helps connect the "dots of care."

Call one of the program's four regional offices for information about the program. You may also contact one of the offices to request:
- Care coordination services - Care coordination services are provided by the CSHS nurses, social workers, and dietitians at no cost to you. CSHS staff will discuss with you how they can help coordinate the services needed for your child.
- Financial assistance - Applications to apply for assistance and more information about that assistance are available from the local offices. You will need to complete the application and provide proof of your current income. Your most recent 1040 tax form is preferred. However, if your income has changed since you filed income tax, the program will accept paycheck stubs showing your income for one month. Your child must meet all program eligibility requirements to qualify for financial assistance. (Your child may also be eligible for South Dakota's Children's Health Insurance Program, administered by the Department of Social Services.) - Assistance with transition planning. CSHS helps prepare your child for transitions at daycare, school, and into adulthood.
- Assistance with problem solving. The CSHS team helps your family identify the best options for your particular situation/needs.
- Advocacy assistance. CSHS helps families access and utilize specialty health care, education, and community resources.
