The application must be completed in its entirety and submitted with the appropriate fee or it will be rejected.

Applicant Initiated Licensure Threshold Requirements:

  1. Complete application form and mail with $75 non-refundable fee to Board office;
  2. Request PA Program Verification of Graduation to be sent from program directly to Board office
  3. If you have other licenses, permits or certificates, request Verification of Licensure to be sent from other states directly to Board office. There is generally a fee required by that state board to order this.
  4. Request PANCE Scores from NCCPA to be sent from NCCPA directly to the Board office

Additional Applicant Requirements: once the application is received, you will receive instructions in writing.

At discretion of Board, these may be required for Physician Assistant Licensure and the applicant will be informed in writing:

  1. Applicant in person interview at the Board office or with a Physician Assistant Advisory Committee (PAAC) member
  2. Signed affidavit in event that practice agreement not available prior to licensure
