Occupational Therapy Assistant (OTA)

TO: Applicants

The application must be completed in its entirety and submitted with the appropriate fee or it will be returned.

The applicant is responsible to request verifications from each state board, the college of graduation and all other required documentation.

Occupational Therapy = Documentation CANNOT be faxed or emailed

Endorsement

  1. Complete application form and file at Board office;
  2. Submit fee of $50;
  3. Submit  Verification of Employment form;
  4. Submit Verification of Licensure form;
  5. Submit Verification of Graduation from Occupational Therapy Program (complete online and print for mailing);
  6. Verification of Certification must be received directly from the National Board for Certification in Occupational Therapy.
  7. Verify all licenses currently or previously held.
  8. Submit a letter from your program director stating that you have successfully completed six months supervised fieldwork experience.

Limited license

  1. Complete form and file at Board office;
  2. Submit fee of $25;
  3. Submit  Verification of Employment form;
  4. Submit Verification of Licensure form;
  5. Submit Verification of Graduation from Occupational Therapy Program; (complete online and print for mailing)
  6. Submit evidence that you have been accepted to write the exam administered by the National Board for Certification in Occupational Therapy and the date scheduled for this exam;
  7. Submit a letter from your program director stating that you have successfully completed six months supervised fieldwork experience;
  8. Submit statement including name and address of person or institution employing the applicant during the time of a limited license.

Endorsement

  2. Submit fee of $50.
  3. Submit  Verification of Employment form.
  4. Submit Verification of Licensure form.
  5. Submit Verification of Graduation from Occupational Therapy Assistant Program. (complete online & print for mailing)
  6. Verification of Certification must be received directly from the National Board for Certification in Occupational Therapy (NBCOT form).
  7. Verify all licenses currently or previously held.
  8. Submit a letter from your program director stating that you have successfully completed two months supervised fieldwork experience.
