Medical Assistant (MA)

Follow the instructions completely.  Your application must be completed in its entirety and submitted with your $10 fee or it will be returned to you.

Medical Assistant Registration

  1. Complete application form and file at Board office.
  2. Submit $10 fee.
  3. Submit a copy of your diploma from your medical assistant program confirming that you graduated from an accredited medical assistant program.
