TO: Applicants for EMERGENCY MEDICAL TECHNICIAN

Instructions must be followed completely.  Your application must be completed in its entirety and submitted with the appropriate fee or it will be returned to you.

The Board must receive verifications from every state board where you have been licensed prior to issuance of a certificate.  You are responsible to send this form which is enclosed to each state board.

Special Skills and Paramedics

  1. Reciprocity - $75
  2. Exam - $50 (exams are given on an "as needed" basis)
  3. Include the certificate from the National Registry and the exam grades or the certificate from another state along with those exam grades.

Intermediate Skills

Renewal for special skills and paramedics - $25 fee annually due by July 15 of each year.  Along with the fee, each applicant is required to provide proof of successful completion of thirty (30) hours of continuing education during the past twelve (12) months.

Renewal for intermediate skills - $25 annually due by July 15 of each year.  Along with the fee, each applicant is required to provide proof of twenty (20) hours of refresher training during the past year.  (This training is to include at least five (5) hours related to ALS training with three (3) hours on airway care, and two (2) hours on IV and fluid therapy, with at least four (4) verified IV’s.)
